Thank you for your interest in the Academy of Fine Arts facility. In preparation for booking an event at our Center, we ask that you complete this application as thoroughly as possible. Please remember to return the completed form to the Academy of Fine Arts no later than 90 days prior to the date of your event.

NOTE: No nails, hooks, tacks, screws, confetti, glitter, birdseed, rice, etc. may be used on any surface or furnishings. No tape or other material can be applied to walls, surfaces, or furnishings. User will be held financially responsible for repair/replacement of any damaged or defaced property.
NOTE: Balloons may be used for decoration as long as they are tied securely.
General Policies:
Please be aware of the following general policies:
  • Non-Smoking Facility: Smoking is prohibited in all buildings of the AFA.
  • Adequate supervision: Children must be under adult supervision at all times. Please allow 1 adult supervisor for each 8 children under the age of 12.
